« on: January 03, 2006, 09:58:28 PM »
The investigation after Bow Mariner exploded is finished. The Captain of Bow Mariner is the guilty one, according to the report. The reason why Bow Mariner exploded was missing safety-routines. Read more about it here:

"Bow Mariner" casualty caused by a "stunning breach of safe practices" from the vessel's Greek managers and crew, says US Coast Guard.

Just completed the report reading (reports available for download from USCG web site). Sad conclusion, this is a good exemple of procedures that stayed on the paper and were not applied. The surviving third Officier did what he was trained for at the best of his knowledge. ISM code is often quoted as ''ballast papers'', sadly this is true when someone onboard or at the office decide it is...and work to prove it. This case is a hard and costy lesson to officers and operators everywhere in the world.   :-(

You can found it in english, on the USCG web site:

Despite the sadness of this case, have a good weekend.
