Just completed the report reading (reports available for download from USCG web site). Sad conclusion, this is a good exemple of procedures that stayed on the paper and were not applied. The surviving third Officier did what he was trained for at the best of his knowledge. ISM code is often quoted as ''ballast papers'', sadly this is true when someone onboard or at the office decide it is...and work to prove it. This case is a hard and costy lesson to officers and operators everywhere in the world. :-(
