Just to update this tragic event - the Marine Accident Investigation Branch (UK) have recently released their report into the accident and this can be read @

The synopsis being:

On 19 December 2007, the tug Flying Phantom was girted and sank,
while acting as a bow tug. She was assisting the bulk carrier Red
