The german BSU (Federal Bureau of Maritime Casualty Investigation) has published the investigation report regarding the collision between the MT CORAL IVORY and MV SIDERFLY on 28 October 2013 in the Kiel Canal.

Oh dear.  Collisions between two vessels under pilotage (where there is no mechanical breakdown) are relatively rare - but when they happen we are reminded that pilots are human and occasionally act recklessly - in this case directly navigating contrary to instructions from VTS.  Just a little counterbalance to the recent thread: http://forum.shipspotting.com/index.php/topic,15009.0.html
